What does it mean when a team takes a 30 sec. timeout and then the PA announcer says extended to a full?

if a timeout, 30 or full, is called within the 30 second window.....ie.....16:18. that timeout becomes a media timeout that takes place of the first break under 16,12,8,4 minutes of each half. Also the first called timeout of the second half, reguardless of time or duration, becomes a media timeout.
