I live within 10 minutes of three different ones. Taxes aren't charged on medical to the purchaser in WV. And they definitely compete for business like crazy. The last place gave me a 50% discount & my next is 30% off.
Delivery isn't allowed in WV (yet) but I do think an exception should be made for terminally ill individuals. Any individual with a severely compromised immune system doesn't need to take any additional risks.

What color is the sky in the world you reside? Do you seriously believe that people that are shooting heroin between their toes are concerned with whether or not the needle is clean? News flash, they are not. They simply pass the needle to the user sitting right next to them. Additionally, the word "exchange" in these programs is a fallacy. There is no requirement to bring any used needles in to receive new ones. Needle litter, and the hazards they bring, are a serious problem where these "exchanges" operate.

Charleston's needle exchange was horribly run & used needles were effing everywhere. I'm not completely against the program but the whole point is have someone interact to try & counsel them when they would exchange the needles. Either the addicts were gaming the system or the system itself was dysfunctional.
